Camp Clover Referrals

Thank you for considering our day programs for your youth or young adult!  Here you will find information to submit referrals for your client, or a young adult you are guardian of.

Camp Clover Respite Day Camp: 4th-8th Grades

Referrals can only be submitted by a Rock County Children's Long-Term Support case worker. Please click the button below to submit a referral.

Camp Clover Day Service Program: 9th-12th Grades

Referrals can only be submitted by a Rock County Children's Long-Term Support case worker. It is for adolescents who are still in secondary school. If you have an 18 year old who has graduated secondary school, please refer to our Young Adult services. Please click the button below to submit a referral.

Camp Clover Day Service Program: Ages 18-25

Referrals can be submitted by a long-term care worker or by a parent/legal guardian for participants who are enrolled in CLTS or IRIS programming at this time. Please click the button below to submit a referral.
